Other Consular Services

I - Documents Required
 
 
1) Previous passport, about to expire or have at least 6 months remaining validity.
 
2) "Recibo de Entrega de Requerimento - RER", signed by the applicant.
Notice: to obtain the RER the applicant must:
a) Click here to fill out the form, click on "Solicitar Passaporte Comum - PACOM" and fill out the Passport Application Form;
b) Print and sign the RER;
c) Passports requested by mail will have a maximum three years validity;
d) In case the applicant is unable to come to the Consulate General in Chicago, his/her signature on the RER must be notarized by a Notary Public;
 
3) One picture, recently taken, size 2”x2” (5x5cm), full face, on a white or clear background.  Instant, digital, photocopies or scanned photos are not accepted.
 
4) Proof of Brazilian Citizenship (including for those who lost their Brazilian passport or whose previous Brazilian passport has been expired for six months or longer).
I) A copy of the Brazilian birth certificate, or of the Brazilian marriage certificate, as well as that of a Brazilian photo ID is accepted.
II) The presentation of the original documents along with their copies for conference is mandatory.
 
Note: In the lack of the original Brazilian documents their copies must be authenticated by a Brazilian or a US Notary Public. Authentications by a US Notary Public
 
5) Valid Brazilian voting registration ("Título de Eleitor").
Expatriates are exempt from presenting the "Título de Eleitor".
 
6) Military Draft, for men between 18 and 45 years of age. 

7) In case of passport loss: Police Report or "Lost, Stolen or Destroyed Travel Document Form" ( Formulário dePerda, Furto, Extravio ou Destruição de Documento de Viagem”).
 
Notice: According to the Brazilian legislation the Consulate General will cancel alllost and found passports.
 
8) Consular fee:
a) US$   80.00 for a new passport;
b) US$ 160.00 for replacement of lost passport.  
 

II - Method of payment
 
 
The Consulate General in Chicago ONLY accepts payments with Money Order issued by the U.S. Postal Service payable to “Consulate General of Brazil in Chicago”.

 
 

III - Processing Time
 
Four (4) working days from the reception of the complete required documentation;
Processing time can be extended without previous notice during high season.
