Passaporte para MENORES de 18 anos

 
I - Documents Required
 
1) Previous passport (when not applying for the first time), having no more than six months remaining validity.
 
2) Birth Certificate
a.       Applicants born in Brazil: present Brazilian Birth Certificate.
b.      Applicants born overseas and registered at the Brazilian Consulate: present the Certificate of Birth Registration.
c.       Applicants born in the US with at least one Brazilian parent who have not been registered at a Brazilian Consulate may present their U.S. Birth Certificate.
 
3) DELIVERY RECEIPT ("Recibo de Entrega de Requerimento”- RER) printed upon completion of ONLINE PASSPORT REQUEST FORM
 
1.       Complete the ONLINE PASSPORT REQUEST FORM by clicking HERE and following instructions (in Portuguese). Please disregard the message about the website's security certificate (see the image after these instructions). 
 
2.       Print and sign the DELIVERY RECEIPT (“Recibo de Entrega de Requerimento”- RER) by visiting THIS web site and providing the required information.
 
3.       The RER must be signed by both of the minor’s parents or legal guardians. Legal guardians must also provide legal proof of guardianship.
 
4.       For passport applications sent by mail, the signature of both parents or legal guardians must be notarized by a notary public. If one parent or legal guardian is delivering the application in person, the signature of the other parent or legal guardian must be notarized by a notary public.
 
4) One photograph
2”x2” (5x5cm), in color or black and white, full-face only, on a white or clear background. Instant, digital, scanned or photocopied photos are not acceptable.
 
5) ID of parent(s) or legal guardian(s)
Both parents or legal guardians must present a photo ID. The Brazilian parent(s) or legal guardian(s) must present a Brazilian photo ID.
 
6) Military Draft Certificate (“Certificado de Alistamento Militar”), only for young men who will reach 18 years of age within the calendar year, proving that applicant is current with his military obligations.
 
7) Authorization for passaport issuance for minors, CLICK HERE for form.
The Authorization can be signed in front of a Consular officer. In this case, the parent must either present an original Brazilian ID, passport, or RNE card, OR, the Authorization can be previously signed, notarized by a notary public.
 
8) In case of previous passport loss: Police Report or "Lost, Stolen or Destroyed Travel Document Form" (Click here to fill out the form).
 
NOTE: In accordance with Brazilian legislation, the Consulate-General will cancel all lost and found passports.
 
9) Consular fee
a.       US $80.00 for a new passport.
b.      US $160.00 for replacement of lost passport.
  

 
II - Method of payment
 
The Consulate-General of Brazil in Chicago ONLY accepts payments in the form of a Money Order issued by the United States Postal Service (USPS), payable to “Consulate-General of Brazil in Chicago.” Personal checks, credit cards, or any other forms of payment are not accepted.
 
 

III - Passport validity
 
a) Passports for minors between 0 and 2 years of age will be valid for 2 years.
b) Passports for minors between 2 and 4 years of age will be valid for 4 years.
c) Passports for minors above 4 years of age will be valid for 5 years.
d) Passports for minors above 13 years of age who were born overseas, and whose birth has not registered at a Brazilian Consulate or at a Registrar (“cartório”) in Brazil, will only be valid until the minor reaches 18 years of age.
 
PLEASE NOTE: Passports requested by mail will have maximum three (3) years validity.
 
 

IV - Processing Time
 
Four (4) working days after the reception of all required documentation. The processing time may increase without notice in times considered high season.
